We do not store customer credit card details

Stripe Checkout and Paypal acts as data controller in regards to your payment instrument information, see their privacy policy (Stripe) & privacy policy (PayPal) for more information about the processing. Stripe will store certain identifying information, such as a password, your email address, or your mobile phone number (“Stripe Credentials”), and your Payment Credentials.

The data we collect about you

Personal data, or personal information, means any information about an individual from which that person can be identified. It does not include data where the identity has been removed (anonymous data).

We may collect, use and retain different categories of personal data about you which we have grouped together follows:

• Identity Data includes first name, last name, former names, username or similar identifier, NHS number, social media identifiers, marital status, title, date of birth and gender.
• Contact Data includes billing address, delivery address, home address, email address and telephone numbers. We may also ask you for an emergency contact number.

If You Fail To Provide Personal Data

Where we need to collect personal data by law or under the terms of a contract we have with you and you fail to provide that data when requested, we may not be able to perform the contract we have or are trying to enter into with you (for example, to provide you with goods or services). In this case, we may have to cancel a product or service you have with us but we will notify you if this is the case at the time.

Disclosures of your personal data

• Authorities: HM Revenue & Customs, The Care Quality Commission and other regulators and other authorities who require reporting of processing activities in certain circumstances.
